The same reading, at home and at the clinic

About the project
Metabolix is a remote monitoring platform for weight-loss clinics: an app that collects a patient’s readings at home, and a dashboard where the care team reads them.
Between visits, a Metabolix patient steps on a scale, checks their blood pressure, wears a glucose sensor or a watch, and may take a sleep apnea test overnight. The clinic enrols them, ships the devices and writes the prescriptions. Metabolix already had a working platform behind all of this. What it needed was a product that patients could find their way around alone, and that a busy care team could read at a glance.
We redesigned both sides. The patient app, on iOS and Android, from the first sign-in to pairing devices, daily tracking, refills and messages. And the clinic’s web app, for monitoring patients, e-prescribing, dispensing, device orders and stock. More than 330 screens went to Metabolix’s developers, with a guide that maps every patient screen to its feature.

The challenge
Patients set it up alone. Clinicians have minutes.
Nobody signs up for Metabolix. A physician enrols the patient, the clinic ships the devices, and an email arrives with a username and a temporary password. After that, the patient is on their own. There is no one to call about pairing a scale or wearing a sleep test, so the app has to do that job, and stay useful for someone who has one device as much as for someone who has five.
On the other side, a clinician has a few minutes a month for each patient, and the dashboard counts them. Every reading has to arrive already sorted: what is normal, what has changed, what needs a call. And a number has to mean the same thing on the patient’s phone as on the clinic’s screen, or the two stop talking about the same thing.

The product
Measure, sync, review, refill
Two apps around one patient. What the devices record at home is what the care team reads at the clinic, in the same words and the same colours.
A health score out of 100, the latest weight, blood pressure, glucose and calories, and the week’s trends, with small missions to keep going. Each device has its own screen: body composition from the scale, blood pressure and glucose with their ranges, watch data, the sleep test report. Refills, nutrition and messages to the clinic sit one tap away.
The app opens with the devices the clinic shipped already listed, each with its own steps: a Wi-Fi or Bluetooth guide for the scale, a PIN for the watch, Health Connect for other wearables. The cellular scale and cuff need nothing at all, so they are never asked to. Anything can be skipped and finished later.
Patients come in two lists, high priority and standard, each with a tab for every service they are enrolled in. Trends, monthly comparisons, alerts to acknowledge, doctor notes and the message thread sit beside the readings, and a timer in the header counts the minutes spent against the monthly limit.
E-prescribing starts with a patient lookup, so nobody is entered twice. Refill requests are approved or denied in a click, and dispensing prints the label, then hands the screen to the patient to review and sign. Behind it all: device orders, physicians, medications, stock levels and shipments for every office.
The idea
Set up by the clinic, finished at home
The clinic has already done half the work, so the app starts from there: the right devices, in order, with a plain next step for each. A patient with only a scale sees a scale. Nothing waits on a device they do not have.
One reading, one meaning
Low, normal, elevated, high: the same bands and the same colours on the phone and on the dashboard. A reading flagged at home is flagged in the clinic, so the patient and the care team are always looking at the same thing.
Minutes go where they matter
Nothing loads until a patient is chosen, so review time is never spent by accident. High-priority patients come first, alerts wait to be acknowledged, and the dashboard says when a patient’s minutes for the month are used up.
Helpful, never a diagnosis
Health data is shared only after the patient accepts the HIPAA terms. The AI coach answers questions about food, sleep and stress without reading anyone’s records, and says it helps you learn, not treat. Messages to the clinic are for non-urgent questions, and the app says what to do in an emergency.

Impact
One system for the patient and the care team
Metabolix went to its developers with both sides designed as one system: a patient app that sets itself up around the devices the clinic shipped, and a dashboard that shows the care team who needs attention first. We started from a user flow and 32 questions for the Metabolix team, and finished with a developer guide and store previews for iPhone, iPad and Android.
